Summary
Clinical Provide advanced assessment, diagnosis, and treatment planning for patients with post-infection conditions. Offer expert clinical advice and support across services. Maintain professional accountability and ensure high standards of care across the service. Contribute to MDT discussions and case reviews, supporting integrated care planning. Promote self-management and patient education strategies to enhance independence and reduce hospital admissions. Leadership & Management Lead the operational delivery of the community PIC service, including workforce planning, caseload management, and rota coordination. Provide day-to-day line management, clinical supervision, and appraisals for team members. Use complex problem-solving and conflict resolution skills to manage team dynamics and service pressures effectively. Participate in leadership activities with the other Long Term Conditions Service Leads
