Summary
To provide and deliver specialist nursing interventions for a caseload of young people with complex needs and their families. To support colleagues within the crisis pathway byprovidingspecialist advice,guidanceand active involvement in cases where there isa high levelof complexity, risk and needidentified. To undertake clinical on call duties in each roster period as agreed (approximately 2 3 per period). To work alongside the lead clinicians within the pathway to ensure fully integrated multi-disciplinary approach and holistic,person centredcare planning underpins approaches and intervention. To work closely with acute general hospital departments/colleagues to ensure high quality in-reach nursing and clinical support is undertaken for young people. To ensure nursing principles underpin and contribute to thedevelopment andimplementation of crisis, home treatment careplanningand intervention To take lead role in the continuous development,maintenanceand review ofhigh qualitysafety and coping plans. To support the on-going development ofhigh qualityrisk assessment/managementand formulation within the crisis pathway To provide specialist advice and support in relation to physical monitoring and management of young people presenting in crisis with restricted eating
