Our ALS Department is expanding and we are excited to announce that we are looking for an additional team member to join as a EHCP
Transition Officer.




We are seeking a passionate and experienced candidate to join our dedicated team.

The Role




Monday to Thursday -Term Time Only - 38 weeks 30 hours per week.




To ensure that effective transition programmes are implemented and monitored relating to the statutory assessment Education,
Health and Care Plans (EHCP) for students with and without special educational needs and/or disability as specified in the Special
Educational Needs and Disability (SEND) Code of Practice (2015).

The successful candidate will also be required to deliver specialist information, advice and guidance to young people and their
parents and carers in a range of external and internal settings.




Main Duties for this role include




* To work collaboratively with students who are transitioning from Specialist Provisions, Alternative Provisions, Resource
Provisions and students who are NEET and at risk of becoming NEET.




* To liaise with Local Authorities/Schools and other educational settings. Join statutory meetings with parents/carers and young
people and attend annual reviews on and off site.




* To be the colleges first point of contact when liaising with agencies, parents, and carers regarding the transition of EHCP
Student’s




* Formulate, implement, monitor and review transition plans.




* Arrange transition visits for potential EHCP students.
