Summary

Management and Leadership Act as a clinical leader in the delivery of nursing services across the Practice; acting as accountable clinical nursing lead in the care provided to patients by the nursing team As part of the senior management team, develop close relationships with partners, colleagues and other members of the team. Providing effective line management to practice nurses and Health Care Assistants. Actively provide the nursing perspective with attendance to relevant internal and external committees or groups Actively maintain, monitor and audit the quality of care provided, with reference to clinical governance, risk or issues management, documentation and patient records Work with the Partnership and Clinicare Health Support Services management team in clinical service redesign and other transformation workstreams as appropriate Ensure that there is effective communication between the nursing team and other teams within the practice Implement evidence-based recommendations and innovations to improve care Actively take part in the recruitment, selection, training and appraisal of practice based nursing posts. Be involved in the teaching, mentoring and coaching of staff members, nursing students and members of the multidisciplinary team as appropriate. Set up, maintain and organise agreed training and development programmes in line with Practice priorities Develop and embed a model of clinical supervision and reflective practice on an individual or group basis. Support HR escalation of performance issues within wider nursing team as appropriate. Be the practice representative for nursing team for the West Leeds Primary Care Network or other external agencies as appropriate Support the ongoing development of the multi-disciplinary clinical team across the practice that will enable provision of innovative, high quality services to our patients To support and influence the role of our nursing team within any clinical service redesign plans both within the Practice and also within the wider Pudsey Primary Care Network Provide leadership in areas related to infection prevention and control and emergency equipment, including being the named Infection Prevention and Control lead for the Practice, ensuring compliance and oversees audit as necessary The practice is committed to supporting and promoting opportunities for staff to maintain their health, well-being and safety. Clinical Duties The content of clinical duties will depend on the specific skills, knowledge and experience of the Lead Nurse. This can include Case management of long term medical conditions in a practice setting, providing subject matter expert advice to patients, evaluating individual specialised treatment plans and referring patients to secondary or specialist care as required Identifying, managing and supporting patients at risk of developing long-term conditions, preventing adverse effects on the patients health Practice Nursing Leading on quality and performance of services that are primarily provided and monitored by the Nursing Team e.g. immunisations, cervical screening, flu, among others, with emphasis on CQC related improvement Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, SIGN, BTS, NICE Maintain accurate clinical records, ensuring effective use of Read codes Maintain chronic disease registers Support the team in dealing with clinical emergencies Recognise, assess, and refer patients presenting with mental health needs Implement vaccination programmes for adults and children Support patients in the use of their prescribed medicines or over the counter medicines within own scope of practice, reviewing annually as required Contribute to practice targets QOF etc, complying with local and regional guidance Liaise with external services and agencies to ensure vulnerable patients are supported appropriately Delegate clinical responsibilities appropriately ensuring safe practice and the task is within the scope of practice of the individual Support the clinical team with all safeguarding matters, in accordance with local and national policies Service Development and Quality Improvement Work with the nursing, clinical and management teams to review the delivery of care within the practice and emerging initiatives including sharing learning and identifying and disseminating best practice Identify areas of service development and act as a lead change agent in implementing quality improvements within the practice clinical team. Openly question and challenge nursing practice in a constructive way so that standards or patient care are continually evaluated and improved, embrace new ideas and make changes for the benefit of patient care. Contribute to and comment on practice aims and objectives, including any business planning process as required. Actively contribute to the monitoring and actions associated with risks and issues related to nursing as part of the practices Assurance Framework Contribute to the effectiveness of the team by reflecting on own and teams performance and making suggestions to improve and enhance activities. Strive to continuously improve quality within the practice Develop and embed a culture of service and quality improvement across the clinical team Personal Development and Professional Responsibilities Actively participate in regular clinical meetings with the partners to review personal development and team performance. Provide a high standard of professional conduct and nursing care always in accordance to the NMC Professional Code of Conduct. Support HR in ensuring all nursing staff records are accurate and complete in line with practice policy and NMC guidance. Monitor effectiveness of own clinical practice through audit and peer-review, with support of Senior GP Partner Actively participate in and promote to other staff, the practices appraisal scheme Take responsibility for own developmental learning and performance Identify appropriate areas of personal development Formulate a personal development plan to meet the criteria of the NMC Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in aspects of the role is maintained Undertake training as required including mandatory and statutory training, core skills training and on-going developmental training. Alert management and other team members to issues of quality and risk, taking action where appropriate Work effectively with individuals in external agencies to meet patient needs. Ensure personal up-to-date validation. Effectively prioritise, organise and manage own time, workload and resources. Be familiar with and adhere to the practice safeguarding policies, procedures and guidance for both children and adults at risk of abuse or neglect. This must be in conjunction with the Multi agency Safeguarding Children and Safeguarding Adults at risk of abuse or neglect Policy, protocols, and guidelines. All employees should undertake mandatory safeguarding children, adults and MCA training as appropriate to role including updates and other professional development activities to ensure training compliancy.
