Summary
The post holder will exercise professional judgement and make autonomous clinical decisions in relation to presenting problems, whether self-referred or referred by other healthcare professionals within the organisation. This includes assessing and managing patients with undifferentiated and undiagnosed conditions. Working collaboratively with patients and in line with current practice disease management protocols, the post holder will develop, implement and review appropriate care plans. The post holder will maintain clear, accurate and contemporaneous clinical records in accordance with agreed standards, compile and issue computer-generated acute and repeat prescriptions, and prescribe in line with the practice formulary, using generic prescribing where clinically appropriate.
