Summary
Act as an autonomous practitioner to provide assessment, advice and liaison within University Hospital Coventry and Warwickshire NHS Trust hospitals supporting people with suspected or diagnosed dementia, delirium, who have been admitted to hospital for a primary physical health reason. Liaise with colleagues and provide education, support and advice to staff within the hospital setting on a range of subjects aimed at improving their understanding and management of dementia, delirium and any associated behavioural and psychological symptoms. Provide specialist advice on care planning and interventions in relation to individual patient care to staff as well as informal carers. Provide advice and support on management in challenging clinical situations and develop specialised care plans e.g. Assessment and management of patients with behaviours which challenge. Work strictly within the framework of the Mental Capacity Act to ensure individuals rights to make their own decisions are always respected and to fully support those individuals who are deemed to lack capacity by contributing to any best interests plans. Act as a clinical role model for effective clinical leadership enabling and facilitating others to take initiatives forward Always maintain a professional attitude, developing the role of the nurse in the best interests of the patient and profession in accordance with the Nursing and midwifery (NMC) Code of Professional Conduct For further details of the role please see the attached job description.
