Summary


Patient Safety and Systems Manager

Band 8A, Fixed Term 6 months

We are currently recruiting a Patient Safety and Systems manager to providing leadership and expertise for the management of patient safety team within our wider Quality team.

The post holder will provide Trust wide oversight, assuring the quality and effectiveness of all types of patient safety incidents and investigations including engagement and the completion of Duty of Candour as part of the Trust’s Patient Safety Incident Response Plan (PSIRP).This post holder will be a Trust specialist on the Radar system including supporting the development and management of events within the system.

Proposed Interview Date - 24th March 2026

· Provide expert advice, guidance and support to all staff on matters related to patient safetyincidents, investigations and learning responses.

Ensure compliance at all times with local and nationalguidance.

Oversight and management of the patient safety team

The identification of systems learning from a patient safety incident that will then guide sustainable risk reduction and service improvement, ensuring that a Just and Learning Culture and human factors principles are maintained.

· Promoting a proactive, positive, evidenced-based and Trust wide approach to gathering and acting upon patient safety and quality of care information. Ensuring that staff, people who use our services, their families, carers and others receive a response to incidents, which is efficient, effective and insightful of the impact across all aspects of trust business.

· Ensuring the timely and effective investigation of incidents and complex complaints, ensuring that regulatory duties toward duty of candour are completed, family concerns are included with terms of reference and supporting the review of the report via an internal expert panel. Effectively manage a Trust-wide electronic reporting system to meet the Strategic needs of the

organisation.

· To manage the monitoring, review and development of all events in the Radar management system
