Summary
Able to formulate appropriate management plans for patients, whilst continuouslyrevaluating initial findings and clinical response. Able to assess new referrals and establish the level of support the individual mayrequire from the service. Is able to undertake a physical examination assessment as indicated by the patientscondition, formulating, implementing and evaluating treatment plans for individuals,families and carers Prescribing and deprescribing of medications for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence based practice and national andlocal protocols and within the roles scope of practice and legal framework Is able to take the lead in complex case management, providing support and adviceto others
