Summary

Job Purpose To act as lead dietitian for the dietetic management of Type 1 & 2 Diabetes and related co-morbidities To act as lead dietician providing expertise within the community specialist diabetes nursing team and to other healthcare professionals within Nottingham City in relation to nutrition and diabetes advice. To support strategic developments within the community diabetes nursing team and the Nottingham City Diabetes Service To maintain, develop and provide education and training on diabetes and related co- morbidities to health care professionals, other relevant professions, students, patients and the public. Dimensions To be the dietetic lead within the integrated community diabetes team providing knowledge and expertise for evidence-based dietary guidelines, practice and education, working within the service as appropriate. To work collaboratively and across organisational boundaries to provide evidence-based practice and holistic care to people with diabetes in a variety of settings. Support diabetes patients registered with Nottingham City GP Practices, as defined by inclusion criteria for the service. To offer a specialist resource to health care professionals working in Nottingham City. Key Responsibilities Clinical Receiving, interpreting and prioritising relevant referrals from a variety of health and social service professionals for patients with diabetes To be an expert source of knowledge, therapeutic dietary advice and treatment recommendations for patients with diabetes and health professionals To develop individual patient centred care plans based on nutritional requirements including interpretation of biochemistry, anthropometry, clinical conditions, medication, diet history, family and social history. Provide appropriate dietary counselling, therapeutic, lifestyle advice to patients and carers. This will include domiciliary settings where appropriate To undertake patient facing work within various venues across Nottingham city, including joint clinics with other members of the diabetes team e.g. Consultant and Diabetes Specialist Nurse, this may also include domiciliary visits To deliver DAFNE structured education programme to the citizens of Nottingham City. Attending clinical and multi-disciplinary team meetings as required. Maintaining accurate and comprehensive patient records. Managing own clinical caseload Undertaking data collection e.g. the use of SystmOne and other packages to collect patient data statistics and undertake audit. Be competent in the use of Information Technology and its application to Dietetics; using Microsoft Word, Excel and PowerPoint effectively, as well as Clinical System Programmes for the accurate review, updates and documentation of patient care plans Regularly undertakes performance management data collection and interpretation of the data in accordance with relevant service outcome measures and key performance indicators Education and Training Demonstrate knowledge of national and local guidelines, ensuring the service is fully compliant. To support the wider team in implementing the strategic direction for diabetes. Keep up to date with current evidence-based diabetes dietary knowledge, dietetic practice and the DAFNE curriculum To actively lead on the education and clinical updating of diabetes colleagues and health professionals including dietetic students Identifying the need for, developing, delivering and evaluating diabetes patient education programmes for specialist dietary and lifestyle management of people with Type 1 and 2 diabetes. e.g. Desmond, Dafne. Delivery will include communicating effectively with patients and carers to ensure understanding of the condition in one-to-one or group education settings. Developing, organising and delivering evidence-based training, incorporating the provision of relevant educational materials and evaluation. Empowering front line professionals to give dietary and lifestyle modification advice to their patients with diabetes. Responding to requests from relevant community groups and patient support groups for dietary education on the prevention and management of diabetes and related co morbidities. Support the ongoing professional development of diabetes dietitians/educators and carry out line management Provide clinical supervision and mentoring of dietetic staff and students in collaboration with the community dietitian leads and dietetic service manager Be responsible for managing own professional development and participate in the appraisal process in order to deliver evidence based best practice Professional Advice and Liaison Actively participate in multi-disciplinary and multi-agency work and liaise with all key stakeholders as appropriate e.g. GPs, Non-medical prescribers, Community Matrons, District Nurses, Care Homes staff, Medicines Management and Commissioning teams Working closely with the community diabetes specialist nursing team, primary care teams and other professional groups to ensure consistent, appropriate and evidence based dietary advice for the management of diabetes and related co morbidities Using negotiation and motivational skills, advise patients, community and health professionals to identify and overcome barriers to make changes with regards to diet and diabetes Competence to Practice Meet the requirements of the Health and Care Professions Council and maintain professional registration Work within the Health and Care Professions Council, British Dietetic Association and local codes of conduct Initiate and / or contribute to professional or diabetes nutrition research, evaluation and audit Commit to continuing professional development, including the undertaking of mandatory and essential training and collection of evidence of competency to practice Complete risk management assessments as required. Comply with policies, procedures and statutory regulations Management Lead in the recruitment and selection of clinical staff within the department. Lead with the induction, supervision and individual performance management of clinical and non-clinical staff within the department. Undertaking appraisals, one to ones, sickness and authorise expenses for clinical staff.
