Summary

Communication and relationship skills Internal Promote a multidisciplinary approach to care by ensuring effective communication is maintained between all staff, patients and their families To help guide patients and relatives through the dying process by empathetic listening and being supportive To attend and present patient profiles at case conferences and multidisciplinary team meetings to ensure effective communications with all members of the Hospice inter-disciplinary and primary care teams To facilitate and encourage effective communication between multidisciplinary teams, participate in relevant meetings and share knowledge Support team members who experience a range of emotions from users of service External With other health and social care professional, the wider PHCT, nursing and residential homes, Marie Curie and OOH services. Supporting patient, families, carers, healthcare professionals known to the service in providing reassurance, advice within your competency, referring to the CPCT on call nurse as appropriate. Knowledge, training and experience First level registered nurse Relevant Post Registration, Palliative Care Course (Palliative Care Dip/Degree, ENB 931,237) or willingness to undertake. Facilitating the education and support of qualified and unqualified staff and participating in the development of nursing practice on the unit Analytical and judgement skills Understanding of team working, but also an awareness of the individual needs of each team member To triage out of hours calls from community patients and refer complex issues to on call team Planning and organisational skills Planning and delivery of care to patients admitted to the IPU, supported by senior nursing, medical and wider MDT. To deputise for the senior nurse when required. To motivate and supervise the nursing team in the provision of holistic patient focused specialise palliative care to patients and carers on the IPU. Physical skills To provide physical, emotional and spiritual support to patients and relatives through excellent evidence based end of life care. Verify death and provide emotional and spiritual support to patients, families, staff and other carers to enable them to understand the progression of advanced terminal disease Safely set up and maintain syringe drivers and administer all medications in accordance with Medway Community Healthcare and NMC guidelines To participate in integrated internal rotation of all shifts and departments that requires nursing skills within the wisdom hospice. Responsibility for patient / client care Manage highly charged emotional situations such as verifying death, conducting after death sessions and mortuary duties in dignified and respectful manner Responsibility for policy and service development implementation To assist Ward Manager and Senior Staff to ensure safe working environment for patients, relatives and staff adhering to MCH policies and awareness of legal and ethical responsibilities To ensure records are kept up to date and patients confidentiality safeguarded in accordance with the organisations policies and NMC To keep up to date with all statutory and mandatory training keeping an up to date record of attendance through a learning passport and ESR. Responsibilities for financial and physical resources Assist nurse in charge with on-site responsibility for security, catering, portering and domestic duties of unit out of hours Responsibilities for human resources (HR) Regularly attend Clinical Supervision provided for MCH staff Act as mentor for students Responsibilities for information resources Link nurse for specific area of palliative nursing care Maintain patient confidentiality Responsibilities for research and development (R&D) To maintain own clinical and professional competency, attend educational session provided within the Hospice and externally To identify learning and development needs in the appraisal process Freedom to act Discretionary control and administration of prescribed ranges of medication
