Summary
Provide face-to-face and telephone consultations to patients Deliver structured medication reviews and medicines optimisation Carry out targeted reviews for high-risk and vulnerable patients Reconcile medicines following hospital discharge and clinic appointments Review medication for newly registered patients Implement and maintain robust repeat prescribing systems Lead on safe, effective, and cost-conscious prescribing Review and act upon medication safety alerts Support patients with self-limiting and minor ailments Improve medicines compliance and patient understanding Provide expert medicines advice to clinical and non-clinical staff Act as the practice lead for medicines management Support QOF, Enhanced Services, and PCN targets Contribute to audit, service evaluation, and quality improvement Review and implement NICE, CQC, and clinical guideline updates Provide education and training to the wider clinical team Work collaboratively within the multidisciplinary team Maintain high standards of confidentiality, professionalism, and clinical governance Promote equality, diversity, and inclusion across all aspects of care Adhere to health, safety, fire, and environmental regulations Participate in mandatory training, CPD, and professional development Support continuous service improvement and innovation
