This is an exciting opportunity to become part of a professional, specialist palliative care team, who work alongside members of the wider multidisciplinary team of health and social care colleagues both in the community and the acute care sectors. The Seaton Hospice at Home is an integrated model for palliative and end of life care. we strive to provide the best possible care for all palliative care patient, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their loved ones. This service provides continuity of care throughout the patient's journey, enabling people to die in their preferred place, which is often their home. The Seaton Hospice at Home service is managed by the Royal Devon and Exeter NHS Trust and is funded by the Seaton League of Friends. The service provides specialist palliative care 7 days a week,365 days a year, with shifts covering 08:00-16:00; 10:00-18:00, and 12:00-20:00. After 20:00 hours a Palliative Nurse Specialist will be on call to respond to our patients who need advice or a visit overnight, thus providing specialist end of life care and carer support 24/7, avoiding unnecessary and unwanted hospital admissions. Working Pattern: 30 hours per week. 4 days a week. Shifts vary between 08:00-16:00; 10:00-18:00; 12:00-20:00 with an on call overnight. Pay: The salary advertised for this position reflects the Agenda for Change 2026-7 rates, inclusive of a 3.3% pay award implemented from April 2026.
