Adult Community Review’s Team – Empowering Adults to Live Well and Independently

The Adult Community Services Review Team supports adults with long term and complex needs to help them live as independently as possible.

We work directly with customers in a person led way, carrying out Care Act 2014 reviews to understand their needs, strengths and goals, ensuring ongoing care packages remain appropriate and effective. We take a strength-based approach, focusing on wellbeing, independence and enabling people to achieve outcomes for themselves wherever possible.

As a Social Care Practitioner, you will be well supported by experienced Social Workers and Social Care Practitioners, with ongoing learning and development opportunities.

Your key responsibilities include completing person centred, outcome focused annual reviews and reassessments. You will review care both in person and over the phone, updating support plans so they accurately reflect each customer’s needs. Travel is therefore an essential part of the role.

You will need excellent interpersonal skills and the ability to work collaboratively with customers, families and multiprofessional teams. This is a fast paced role, so strong organisational skills and the ability to manage a caseload confidently are important. Experience of Adult Social Care processes, along with the ability to work with accuracy and efficiency, is essential. NVQ Level 4 in Health and Social Care (or equivalent) is desirable, or a willingness to work towards it.
