Summary
JOB SUMMARY This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed. The content of this post will be reviewed in consultation with the postholder when necessary and in line with the service developments. JOB PURPOSE: To carry out duties in a skilled and competent manner in order to provide an appropriate level of skilled nursing care to clients in the community. The post holder will assess the health needs of designated service users referred to the service and will plan, implement and evaluate programmes of care in response to identified need. CORE DUTIES AND RESPONSIBILITIES: Participate in the delivery of a cardiac / pulmonary rehabilitation programme, along with the multidisciplinary team, supervising exercise to patients in a gym setting. Deliver structured education sessions to groups of patients and their family/carers. Participate in the planning and delivery of packages of care, including liaison with other health professionals and statutory and voluntary agencies to deliver skilled nursing care and support any changes. To be aware of and act in accordance with Trust Clinical Standards and Guidelines and the NMC Code of Conduct and Guidelines maintaining competency and attending training as appropriate. Record all patient contacts and activities accurately and contemporaneously maintaining accurate records and statistical returns as required by the Trust including entering data onto the Trusts systems within the required timeframes. Advise patients on health promotion issues in order to achieve pre-determined local and national health targets. Participate in health screening programmes and disease management of patients under the direction of the Specialist Nurse / Physiotherapist / Clinical Team Leader. Act as a mentor for students and participate in all aspects and levels of training in order to consolidate theoretical and practical teaching of staff and learners. To be a skilled communicator establishing and maintaining excellent relationships with members of the cardiac / pulmonary rehabilitation team and other Health and Social Care professionals, initiating referrals where appropriate to ensure adequate support for patients and carers. To have knowledge of and be able to effectively use available resources. To participate in clinical supervision on a regular basis with a designated person. COMMUNICATION WITH OTHERS: 1. Patients, their carers and families. 2. All members of the cardiac / pulmonary rehabilitation team in order to maintain professional relationships and sharing of information. 3. Colleagues within the larger team to provide workload cover and professional support. 4. Other health professionals (hospital and community), and statutory and voluntary agencies to ensure a team approach to client care. 5. The management team, to provide managerial support and advice in the problem solving process, and to facilitate professional development.
