Summary
The Salaried GP will have both Clinical and Operational responsibilities as outlined below: To independently assess, examine, diagnose and manage patients presenting with a wide range of medical conditions, delivering evidence-based care in both surgery and home visit settings in accordance with current best practice guidelines. To practise autonomously whilst working within the clinical governance framework and strategic direction of the Practice. To make timely and appropriate referrals to secondary care and community services, working collaboratively with multidisciplinary teams and external agencies to ensure safe, coordinated, and patient-centred care. To support shared decision-making by offering patients informed choice and involving them actively in their care planning. To prescribe safely, appropriately, and cost-effectively in line with national guidance, local prescribing formularies, and Practice policies. To maintain accurate, systematic, and contemporaneous clinical records using the Practices electronic clinical system, adhering to agreed coding templates and data quality standards. To participate fully in the investigation and management of complaints and significant events within the designated clinical area, in line with Practice policies and clinical governance frameworks. To report all critical incidents, accidents, and near-misses in a timely manner, and to actively engage in subsequent review processes, action planning, and monitoring to support continuous quality improvement and patient safety. To participate in Individual/Team Performance Review, attend mandatory training courses and complete an Annual Personal Learning Plan to identify individual training and development needs. In addition to routine practice-based clinical sessions, this role offers the opportunity to take a lead or enhanced role in: Proactive care home ward rounds Comprehensive frailty assessments and personalised care planning Structured medication reviews in collaboration with our pharmacy team Advanced care planning and end-of-life care discussions Reducing avoidable hospital admissions through anticipatory care Developing strong working relationships with care home staff and community teams
