Job Title:
Domiciliary & Outreach Care Worker
Location:
Wirral (community-based)
Job Purpose:
To provide high-quality, person-centred care and support to elderly clients and individuals with learning disabilities and mental health needs, enabling them to live as independently and safely as possible within their own homes and the community.
Key Responsibilities:
Provide personal care including washing, dressing, toileting and continence support
Support with medication administration or prompting in line with care plans
Assist with meal preparation, nutrition and hydration
Support with mobility, transfers and use of equipment where required
Carry out light domestic duties such as cleaning, laundry and shopping
Offer emotional support, companionship and reassurance
Support clients to access the community, attend appointments and engage in social activities
Work in line with individual care plans and risk assessments
Monitor and report any changes in a client’s health, behaviour or wellbeing
Maintain accurate records and documentation
Promote dignity, independence, choice and respect at all times
(These duties reflect typical UK domiciliary care roles which include personal care, medication support, domestic tasks and community access support.)
Person Specification (Brief):
Caring, compassionate and reliable
Good communication and interpersonal skills
Ability to work independently and as part of a team
Flexible to work shifts, including evenings and weekends
Experience in care (preferred but not essential)
Full UK driving licence (desirable for community work)
