Summary
To participate pro-actively in the development of an efficient and effective Homeless Health Service.
To ensure the health needs of people experiencing homelessness (both acute and chronic) are met in order to promote wellness and prevent ill health, and early death.
To use advanced nursing skills and knowledge to holistically assess patients’ needs and to provide clinical treatments based on evidence based practice.
To pro-actively engage with underserved groups through outreach to the street and other locations, and liaison with outreach teams and hostel staff.
To advocate for patients who face multiple barriers to accessing healthcare and raise awareness among other healthcare professionals, students and social care staff of the health issues affecting people experiencing homelessness.
• To use advanced nursing skills and knowledge to provide autonomous holistic assessment, diagnosis and management of homeless patients presenting with acute undifferentiated and undiagnosed conditions and injuries, complex health needs and/or long term conditions.
• To provide complete episodes of care based on the best possible evidence and work within clinical guidelines, utilising expert judgement to work outside these frameworks when necessary.
• To provide advanced nursing care to people experiencing homelessness both in a clinic setting and during outreach to the street and other locations.
• To work in partnership with other community teams / voluntary organisations in order to ensure the seamless delivery of care to the homeless population and proactive engagement with underserved groups.
• To act as an advocate for people experiencing homelessness, assisting patients to overcome barriers to healthcare and supporting their own choices in regards to their care whilst promoting independence and self-care.
