Summary
Job Purpose To support the Nutrition and Dietetic Services in the community including care homes and home visits. To support the Dietitians with the Oral Nutritional Supplement service (ONS). To support Dietitians with the delivery of patient group education sessions To support Dietitians with the delivery of nutrition related topics to other professionals To support the Community Diabetes team with Type 2 diabetes structured education Dimensions To support the team to deliver care within job scope and national/local guidelines. This will include collection of anthropometric measurements. Working in collaboration with other professions. Working within legislation, policies, procedures and core values. You will be required to participate actively in City Cares training requirements, including in-service training and multi-agency training. This post has no budgetary responsibility; however, the post holder is responsible for demonstrating cost consciousness. You will be required to work across different bases and community settings which could include clients own home, residential or nursing homes, health centres and other community venues in the Nottingham City area. Key Responsibilities Clinical To undertake individual treatment and interventions with clients under the supervision of qualified staff. To support qualified staff with providing therapeutic dietary and lifestyle advice to patients. To support qualified staff in the Oral Nutritional Supplements (ONS) service by regularly monitoring waiting lists and carrying out patient reviews. To monitor individual patients nutritional intakes and requirements through the evaluation of a complex range of information. This will include assessment of individual nutritional and clinical circumstances, patient priorities, lifestyle, and ability to understand and implement change. To utilise the following skills and competencies for monitoring adult patients. Reviewing anthropometric measurements, appropriateness of, and compliance with nutrition care plans. To communicate relevant dietary advice and lifestyle advice effectively To support qualified staff to provide nutrition and dietetic services to care homes. To support qualified staff to provide a domiciliary service to housebound patients and their carers to review patients. To accept responsibility for designated caseload. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role (as per AP guidelines). To have the ability to gain valid informed consent and to identify through assessment, clients who lack capacity. To work within a legal framework with clients who lack the capacity to consent to treatment. To attend and contribute to team meetings and case discussions using effective communication. To maintain comprehensive and accurate records in line with polices Organisational To participate in the development of good practice for the AP role within Nottingham CityCare Partnership To provide support to other Assistant Practitioners as appropriate. To be responsible for own Continuing Professional Development, incorporating it into your work as necessary. To actively participate in the services training strategy. To use information technology daily and participate in identified data collection and statistics and to maintain clear, concise and timely records in line with the service policy. To always have regard for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties. To actively promote equality and diversity within service delivery and respect peoples individual rights. To liaise effectively with other professionals and agencies. Education and Development To ensure knowledge, skills and competencies are kept up to date. To be involved in organisational appraisal, clinical and management supervision, and be active in identifying aims and objectives for personal development. To assist in the training and development of others (new members of staff, students and visitors to the service). To support the delivery of patient group education sessions (e.g. diabetes education) To support the delivery of evidence-based training on diet and nutrition-related topics, incorporating the provision of relevant educational materials and evaluation. To undertake any other duties that may be reasonably required by clinicians. To actively participate in audit and research. Professional Advice and Liaison Supporting the clinical dietetic team to work with CityCares neighbourhood teams and other professional groups to ensure consistent and appropriate nutrition and dietetic advice is available to the community of Nottingham City.
