Purpose of Role Health & Wellbeing Coaches support people to take pro-active steps to improve the way they manage their physical and mental health based on what matters to them. The service encourages people, through a programme of intensive 1-2-1 support, to develop their knowledge, skills and confidence in managing their health to enable them to improve their health outcomes and quality of life. Comprehensive and holistic assessments of need will be completed in order to collaboratively identify support, goals and opportunities. Health & Wellbeing Coaches will be integrated as a key part of the Primary Care Networks (PCN) multidisciplinary team working alongside Social Prescribing Link Workers and Care Co-ordinators to provide an all-encompassing approach to personalised care. The post holder will provide 1-2-1 and group health coaching support for people with one or more long-term conditions to improve levels of activation enabling self management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual needs. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access. Health & Wellbeing Coaches will build and develop relationships with a wide range of services and activities in order to address social and health care needs, signposting and offering support to individuals to participate and benefit from community based activity. As part of Chapeltown PCNs Wellbeing Team (along with Link Workers and Occupational Therapist) you will review pathways in conjunction with the Service Manager to ensure that provision is of a consistently high quality. Main Duties and Responsibilities 1. To complete comprehensive and holistic assessments of need for those referred to the service within agreed timeframes. Giving people time to focus on what matters to me 2. To manage and prioritise a caseload, in accordance with need, to facilitate programmes of activity that benefit health and wellbeing 3. To triage referrals and signpost to specialist support quickly where necessary 4. Provide 1-2-1 and group health coaching support for people, including those with one or more long term conditions, to self-manage their health and encourage behavioural and lifestyle change 5. To develop collaborative support plans with individuals which promote choice and complement any medical treatment or advice 6. Work as part of a multidisciplinary, multi-agency team to promote health coaching and ensure GPs, practice nurses and pharmacists and other members of the primary care team understand the role and how to refer patients who may benefit from health coaching 7. Attend and contribute to team, practice and PCN meetings and events as required by the service 8. To monitor and record outcomes within SystmOne through the use of recognised assessment tools and outcome measures, including SWEMWBS and the Wellbeing Wheel 9. To gather and collate statistical information as required including both service user experience and impact of embedding personalised care into local health systems 10. To build networks with local community resources such as activities and services that may have an impact on health and wellbeing and support individuals to access these 11. To develop multi-agency working across communities and voluntary and statutory organisations to promote health coaching, its role in self-management and the wider determinants of health 12. To work closely with PCN reviewing pathways in conjunction with the service manager to ensure that provision is of a consistently high quality 13. To reflect on practice and participate in team, practice and PCN meetings 14. Participate in regular health coaching and continual learning 15. To ensure that client records and other information systems are completed accurately and within agreed timescales. 16. To ensure that support undertaken is meaningful, respectful, promotes self-determination and is in line with the individual support plan. 17. To adhere to organisational policies and procedures relating to risk and personal safety 18.
