Summary
To participate in Pharmacy clinical services delivery involving any of the following: ensuring accurate drug histories are taken and documented; medicines reconciliation is completed; patients own drugs are reconciled; dispensing for discharge is facilitated; communicate effectively with patients and/or carers to improve medication adherence. To effectively communicate with other members of the multi-disciplinary team to ensure safe and effective prescribing and administration practice of medicines at the Trust. To identify patients with complex pharmaceutical needs, propose solutions and refer as needed to more senior staff. To monitor patients response to drug treatments for efficacy and toxicity, prioritise these andtake appropriate action to ensure the safe and effective use of medicines. To identify drug interactions, prioritise these and take appropriate action to ensure the safe and effective use of medicines. To identify errors on clerking including medication histories and allergies documentation, TTOs or on other prescriptions initiated during the patients admission. Appropriately prioritise these and take the best course of action to resolve these to ensure patient safety. To identify deviations in prescribing from guidelines or formulary standards and take appropriate action to ensure appropriate prescribing practice is maintained at the Trust. To complete all documentation according to Trust and departmental policies. To work effectively as part of a team in a multi-disciplinary and Pharmacy environment. To undertake clinical audit as required by the department. To undertake peer assessment activities as outlined in the Pharmacy Step Pathway to assess communication and decision-making skills, obtain feedback, and amend practice where necessary. To support with the delivery of training and education of student placements as required by the department. To undertake prescribing responsibilities as an independent prescriber in line with the Trusts Universal Scope of Practice for Pharmacist Independent Prescribing, Standard Operating Procedures, and Clinical Guidelines, or within a defined specialist prescribing scope formally approved through the Medicines Policy and Practice Committee, ensuring safe and effective prescribing practice at all times For further details of the role please see the attached job description.
