Summary
Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health. Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills. Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management. Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys. Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives. Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students. Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults. Maintain an up-to-date professional portfolio to evidence ongoing advanced-level competency
