Summary
This role is responsible for carrying out clinical triage for children and young peoples referrals into specialist services across BaNES, Swindon and Wiltshire, as well as some referrals for the School Aged Health Services in BaNES and Wiltshire. They will ensure all referrals including urgent ones are reviewed within the required timeframes so that referrers, parents/carers, and young people receive clear outcomes, guidance, and waiting well support promptly. In addition, this role will work closely with the Clinical Triage Lead on multidisciplinary triage (MDT) for more complex or multi-need cases and may chair MDT meetings when delegated.They will also support the Care Coordination team by helping respond to clinical queries and by communicating with families, education partners, third-sector organisations, and other professionals in a timely and supportive way. Overall, this role plays a key part in helping families navigate HCRG Care Group services and partner organisations to ensure care is joined-up, holistic, and aligned to a stepped-care approach.
