Summary

As part of the Neonatal Service at NUH, the post holder will actively support families in the care of their baby (who fulfil the stated criteria in the Neonatal Service Home Visiting Guidelines and Neonatal Home Phototherapy Guidelines) both on the neonatal units and transitional care areas. As part of the Neonatal Service at NUH, the post holder will assure that all babies (who fulfil the stated criteria in the Neonatal Service Home Visiting Guidelines) receive access to the home visiting programme after discharge from the neonatal service. The post holder will participate in the discussion around discharge planning with the parents/carers of the baby, the medical team, low dependency team and wider multidisciplinary team prior to discharge of the baby and then facilitate the documented home care plan for this baby at home until such time that the baby is deemed fit to transfer care to the relevant health visiting team and wider community. The post holder will act to ensure that the standards and quality of care given to babies, together with the environment in which care is delivered, are maintained at a high standard. The post holder will be expected to facilitate and co-ordinate home care and family centred care. Candidates will be expected to undertake a regular clinical shift on either unit, equivalent to 10% of their weekly hours.
