Summary
JD attached in documents The post holder will: Provide compassionate, safe and effective evidence-based care and support to people in a range of care settings Deliver high quality, compassionate care with a focus on promoting health andindependence Work as part of a designated clinical and care team providing and monitoring carethat focuses on the direct needs of the individual Maintain a current working knowledge of other providers resources and referralsystems to ensure individuals needs are met, within parameters of practice Work independently in line with the individuals defined plan of care, within theparameters of practice of the Nursing Associate role, accessing clinical and careadvice when needed Implement individualised care plans ensuring all care provided adheres to policiesand procedures. Work in partnership with all members of the multi professional health care team bothwithin and external to the Trust to ensure continuity of care for the patient Promote and develop the use of current evidence to inform the delivery and promotethe development of clinically effective care. Communicate effectively with patients, relatives and other members of the care teamto ensure a shared understanding of the patients care. Undertake a range of clinical procedures relevant to the clinical area.
