Summary
Main duties of the job Deliver evidence-based nursing care within your scope of practice Support long-term condition management of diabetes (and associated conditions) and run clinics. Support public health and screening programmes, including: immunisations & vaccines, cervical screening Health promotion and prevention Provide wound care and routine nursing procedures Contribute to our research programmes Contribute to QOF, enhanced services and practice targets Maintain accurate clinical records Teach and support patients to self-manage their conditions Participate in clinical audits, quality improvement and governance activity Liaise with external services to ensure patients receive appropriate care Respond appropriately to clinical emergencies and incidents Essential Registered Nurse with the NMC Experience of chronic disease management - Diabetes Ability to work autonomously and as part of a multidisciplinary team Excellent communication, organisational and time-management skills Commitment to ongoing professional development and revalidation Desirable Experience working as a Practice Nurse or Community Nurse Strong clinical skills including immunisations & wound care Good understanding of safeguarding adults and children Knowledge of GP systems, QOF and enhanced services Diabetic Diploma or equivalent (training can be provided)
