Summary
Main Duties and Clinical Responsibilities Undertake comprehensive clinical assessments, including history taking, physical examination, and diagnosis Manage undifferentiated and differentiated presentations independently assess, diagnose, treat, and discharge or refer patients as appropriate Prescribe medications independently in line with clinical guidelines and best practice Manage acute, urgent, and long-term conditions Order, interpret, and act upon diagnostic tests and investigations Provide health promotion, disease prevention, and patient education Provide same-day urgent care appointments and routine clinical appointments as required Refer patients to Secondary Care, community services, or allied health professionals where appropriate Ensure continuity of care through appropriate follow-up and safety-netting Maintain accurate, clear, and contemporaneous clinical records Multidisciplinary working -work collaboratively with GPs, practice nurses, HCAs, pharmacists, and the wider PCN team Participate in MDT meetings and clinical discussions Support and mentor junior staff where appropriate Quality, governance, and safety work within practice policies, procedures, and clinical governance frameworks, adhere to safeguarding policies for children and vulnerable adults Participate in clinical audit, quality improvement, and service development initiatives Maintain professional registration, revalidation, and mandatory training Personal Professional Development Maintain and develop clinical competence through CPD Keep up to date with evidence-based practice and national guidance Actively contribute to a learning and supportive practice environment Responsibility for Information Governance Ensure patient confidentiality is maintained at all times comply with GDPR, data protection, and Caldicott principles Use clinical systems appropriately and securely Health & Safety Work in accordance with Health & Safety legislation and practice policies report incidents, near misses, and risks appropriately.
