Summary
This is an exciting opportunity for a qualified school nurse or health visitor who has the transfer fields of practice or is willing to undertake this course or experienced staff nurse.
The role is part of our Inclusion pathway within the Healthy Child Programme Service 0-19 which incorporates Health Visiting, School Nursing and Teenage Pregnancy Services. The Senior Practitioner (School Nursing) - Youth Justice post sits within the Inclusion Pathway HCP 0-19 and is part of the School Nursing provision. The post holder will lead on the provision of the Healthy Child Programme (HCP) to children and young people who are supported within the Youth Justice Service. The post holder will work closely with external colleagues within the Youth Justice Team and internal colleagues delivering care as part of the HCP 0-19 Inclusion Pathway
The School Nursing, Health Visiting and Teenage Pregnancy form the Healthy Child Programme 0-19 which deliver public health services to babies, children and young people and their families. The overarching aim of public health services is to prevent, promote and protect the health and wellbeing of children and families. They work with stakeholders across a number of settings and organisations to lead delivery of the Healthy Child Programme (5-19) to improve child health outcomes and ensure that families in need of help or at risk are identified at the earliest opportunity.
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The £170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. A new Urgent and Emergency Care Centre, opened in March 2023, has significantly improved emergency care facilities and capacity, providing almost 5,000 square metres of additional clinical space.
• To manage your own caseload, identifying and responding to priorities. To plan, develop, implement and evaluate programmes of care.
• Contribute to the development, delivery and evaluation of health education programmes and staff training
• Be able to work in a single point of access to assess new referrals, manage risk, provide parent and professional consultation and provide information to other agencies, in line with information sharing agreements.
• To provide clinical advice and support to the team, provide caseload supervision, 1-1’s and PDR’s for junior staff. Provide learning opportunities, induction, and preceptorship for all learners.
• Contribute to the development of, implement and evaluate new and existing policies ensuring that practice is evidence-based.
• Maintain clinical governance processes, e.g., identification & management of risks and incidents
• To co-ordinate and/or contribute to the health needs assessment of children under the care of the team, participating in the multi-disciplinary and multi-agency assessment & intervention of/for children & young people.
• To provide clinical advice and support, on aspects of care for children & young people.
• Take the lead professional/key worker role for children & young people when appropriate and work as part of multi-agency teams to support those on child protection, child in need and early help plans or those who are looked after.
• To advocate for families within the multidisciplinary team and make referrals as appropriate.
• To work closely with members of the youth justice service, schools and education provisions and provide advice and support to parents and staff where children or young people have identified health needs.
• To have a wide range of knowledge in approaches to communicating and managing the care of children, young people and their parents/carers.
• To be able to effectively communicate with colleagues, peers, senior managers and clinical leads within the Trust.
• To be able to communicate complex patient related information facilitating positive outcomes and ensuring collaborative working.
• To act up in the absence of the SCPHN.
• To always practice within NMC standards and the Code of Conduct
