Based in the Brent Unplanned Care Team at Sudbury
TheHospital at Homeservice is transforming the way we deliver care, bringinghospital-level treatmentdirectly into patients' homes. We provide intensive care for acute conditions that would typically require a hospital bed, offering a short-term, home-based solution through amultidisciplinary teamof expert professionals.
Our mission is tofree up hospital capacityby supporting patients early and preventing unnecessary admissions or attendances. We ensure that patients receive theright care, at the right time, right in the comfort of their own homes. Our team includes specially trainedGPsand healthcare professionals who visit each patient, delivering a wide range of services includingtelemetry, point-of-care testing, andIV therapy, among others.
As anAdvanced Clinical Practitionerin this dynamic, patient-centred team, you’ll play a pivotal role in delivering high-quality care and ensuring a seamless experience for patients transitioning from hospital to home. You'll work collaboratively with a skilled team, utilising advanced diagnostic skills and leading patient care with autonomy and expertise.

• Conduct Comprehensive Assessments: Lead thorough health assessments, including physical examinations, and order necessary diagnostic tests for patients in the Hospital at Home service.
• Advanced Clinical Decision-Making: Use advanced assessment skills to coordinate care, make informed decisions, and initiate referrals for further diagnostics as needed.
• Interpret and Formulate Treatment Plans:Analyse complex diagnostic results, and develop tailored treatment plans that guide both patient care and family involvement.
• Autonomous Patient Care: Act as an autonomous practitioner, assessing, diagnosing, and treating patients efficiently and effectively, ensuring timely interventions.
• Critical Decision-Making: Demonstrate sound clinical judgment in decision-making, ensuring the best outcomes for patients in a dynamic home setting.
• Adhere to Professional Standards: Work within established professional guidelines, codes of conduct, and organizational policies to ensure the highest level of care.
• Patient-Centered Care: Ensure that care is accessible, effective, and tailored to meet the unique needs of each patient and their carers, delivered at the right time and place.
• Support Service Delivery Goals: Contribute to the achievement of key performance indicators, strategic objectives, and continuous service development.
• Manage Day-to-Day Service Delivery:Oversee the quality, risk management, and performance of your caseload, ensuring high-quality care aligned with clinical goals.
