Summary
To monitor and evaluate care programmes for patients and carers, promoting effective co-ordination and continuity of patient care. To communicate formally and informally with the DN team lead to ensure best practice and consistency with the community and intermediate care service in TSDFT An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills To liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problems To undertake Specialist Nursing and Continuing Care assessments (with appropriate training) Please refer to your suitability to the post in your supporting information from the role requirements or person specification.
