Crynodeb
To identify potential risk situations in line with Safeguarding Policy and Procedures and refer these to appropriate agencies and maintaining confidentiality. To keep renal social worker updated of case work and any potential safeguarding concerns identified and risks. To be flexible and carry out home visits to assist patients with completing forms where appropriate. To maintain good communication with colleagues and agencies involved ensuring affective communication of patients needs are maintained. Work collaboratively with other colleagues in the team and wider organisation to enhance the services provided. To maintain accurate documentation keeping patients files up to date and archive file records accordance with Trust policies and procedures. To use computers and other items of technology to carry out work related duties, maintaining both written and computerised records in line with Data protection Act. To participate in appropriate training and development activities to ensure knowledge is up to date. To participate in supervision meetings with the appropriate line manager Participate in appraisals and personal reviews and work to achieve agreed set objectives. Ensure that all Trust wider standards are maintained to improve the quality of care to all who encounter renal services provided by University hospitals Coventry and Warwickshire NHS trust.
