Are you passionate about supporting adults to live well, stay independent, and access the right support at the right time?


We’re looking for a proactive, skilled Wellbeing Assessor to join our fast-paced Front Door Team, the first point of contact for adults seeking advice, information, and social care support.


This is a dynamic role where no two days are the same.


You will speak directly with individuals, families, and professionals to triage needs, gather essential information, and make initial decisions about appropriate next steps.


You’ll work closely with social workers, occupational therapists, and partner agencies to ensure people receive timely, person-centred support.

Key Responsibilities:

Complete strengths-based initial assessments and wellbeing conversations with adults and carers contacting the service
Make on-the-spot decisions about risk, safeguarding concerns, and urgent needs, with access to support and guidance from senior practitioners
Provide advice, signposting, and early intervention options where appropriate.
Identify when cases require escalation for statutory assessments.
Manage a high volume of calls and digital referrals in a fast-paced environment, often handling complex information quickly and accurately
Record information clearly and efficiently on our case management system.
Work collaboratively with internal teams and external partners, including health services, community organisations, and voluntary sector providers.
Contribute to a positive, supportive team culture where learning and development are encouraged.


About You:

We’re looking for someone who is compassionate, confident, and resilient, with excellent judgement and communication skills.
Experience working in adult social care, health, community support, or a similar people-focused environment.
Strong decision-making skills, with the ability to assess risk and prioritise effectively.
The ability to remain calm under pressure and handle sensitive conversations with empathy.
A solid understanding of the Care Act 2014, safeguarding principles, and strengths-based approaches (training offered if needed).
Excellent organisational skills and the ability to balance competing demands.
A collaborative approach and willingness to seek support from senior colleagues when required.
Proficiency in IT systems and confident recording accurate case notes in real time.
