Bydd gennych gyfrifoldeb am achosion pobl ifanc a byddwch yn cyfrannu at y gwaith o asesu a llunio Cynlluniau Llwybr ar gyfer y sawl sy’n gymwys. Mae’r Cynllun Llwybr yn nodi dyheadau’r unigolyn ifanc o ran addysg, llety, iechyd, perthnasoedd cymdeithasol ac anghenion penodol.


Bydd deilydd y swydd yn rheoli llwyth achosion, yn cymryd rhan yn y system ddyletswydd PA ac yn cynorthwyo i wella darpariaeth gwasanaeth.


Oherwydd natur y gwaith, bydd angen cael gwiriad boddhaol gan y Gwasanaeth Datgelu a Gwahardd.

Work base: Coed Pella/ Hybrid


An exciting opportunity has arisen to join Conwy Social Services as a Personal Advisor (PA) dedicated to working with young people from the age of 16-25. You will have case holding responsibility for young people and participate in the assessment and preparation of Pathway Plans for those who are eligible. The Pathway Plan sets out the young person's aspirations regarding education, accommodation, health, social relationships, and specific needs.


The post holder will manage a case load, will take part in the PA duty system and assist with the improvement of the service delivery.


Due to the nature of the work, it will be necessary to obtain a satisfactory disclosure from the Disclosure and Barring Service (DBS)


This role offers flexible working options for a work life balance. This can include adjusting your working day and hybrid working, ie a balance of office and home working.
