We’re looking for dynamic, forward thinking Band 5 Community Staff Nurses to join our Bridlington locality Community Nursing Team within the Integrated Nursing and Conditions Services at CHCP CIC. We currently have a 22.5hr position available. As part of our integrated community nursing team, you’ll deliver high quality, holistic care to people in their own homes, helping them stay well, safe, and supported in their community

This is a great opportunity to build your community nursing skills while supporting people to stay safe and well at home.

You’ll work as part of our District Nursing Service, providing person‑centred care to people with a wide range of health needs. You’ll visit patients in their own homes, helping them manage their conditions and supporting them and their families during times of illness or change. You’ll be part of a supportive team who value good communication, joint decision‑making and high‑quality care.


What you’ll do:
• Carry out holistic assessments and plan safe, effective nursing care
• Deliver a range of clinical interventions, including wound care, medicines support and IV therapies
• Communicate clearly with patients, families and partner agencies to ensure joined‑up care
• Maintain accurate records in line with professional standards
• Support junior staff and contribute to service development
What we’re looking for:
• A Registered Nurse (or awaiting final registration) with a caring, patient‑focused approach
• Strong communication skills, including the ability to handle sensitive situations
• Ability to work independently and as part of a multidisciplinary team
• Willingness to learn new skills, adapt and develop your practice
• Basic IT skills and confidence using electronic records
