Summary
Please refer to the job overview and in addition: Conduct Structured Medication Reviews (SMRs) for patients with complex polypharmacy or long-term conditions. Review and process acute prescription requests, ensuring safe and evidence-based prescribing. Action and manage hospital discharge summaries, outpatient letters and other clinical correspondence. Provide medicines optimisation support, including deprescribing, monitoring, and safety interventions. Support chronic disease reviews as part of the multidisciplinary team (e.g., hypertension, asthma, diabetes, heart failure). Manage patient queries relating to medicines, minor ailments and condition self-management. Provide prescribing advice and guidance to practice teams within your clinical competence. Work towards QOF, IIF and local incentive framework targets as required by the practice. Document all clinical work to a high standard within practice protocols and NHS information governance requirements.
