Summary
As a Support Time and Recovery Worker you will assist in the assessment of care needs and the formulation, implementation and evaluation of programmes of care for individuals (CYP, Adults, and Older persons) carrying out all relevant forms of care under agreed levels of supervision. You will assist in engagement with services and assist in identifying strategies to support mental wellbeing and in the promotion of self-care and social inclusion. You will communicate and work collaboratively with the Mental Health Practitioner and the general practice team to meet the needs of patients. You will support the delivery of timely access to integrated, high quality, service user-centred services in primary care that enable people with mental health needs, physical health needs and/or social care problems to improve their well-being and functionality, thereby preventing, reducing or delaying the need for more specialist services. You will work as part of the integrated primary care team under the supervision of the qualified ARRS Mental health practitioners to ensure that all of the needs of service users are addressed thereby promoting wellbeing, independence and choice whilst preventing, reducing or delaying the greater onset of need whilst adhering to policy and procedures. You will liaise with other professionals and agencies both statutory and non-statutory to provide a link between primary and secondary services from a North Staffs Combined perspective.
