Summary
We are currently seeking a dedicated Clinical Pharmacist (Independent Prescriber) to join our Primary Care Network team . This position offers an exciting opportunity for an individual with a passion for patient care and medication management. Duties: - Action requests for changes in medication and medication reconciliation - Review patient medical histories and medication regimens to ensure safe and effective drug therapy - Undertake medication reviews - Provide drug information and counselling to patients on proper medication use - Conduct medication reconciliation and ensure accurate documentation of pharmaceutical care interventions
