We currently have full time vacancies within our Locality Service for Experienced Social Workers (minimum of 12 months experience). The Locality Teams carry out a care management function, working primarily with people over the age of 65 years with a range of complex needs including Dementia. The service covers four geographical areas in Warrington: Central, East, South, and West. Each team has a Team Manager, a Senior Practitioner, Experienced Social Workers, Social Workers (including ASYE) and Community Care Workers. As a valued member of the team, you will have the support of your line manager with your individual caseload. The teams also have access to administrative support.
We have opportunities for all social workers that will enable you to develop further skills within Warrington's Locality Teams.
There are also potential opportunities to specialise in other areas including learning disability, mental health, physical disability, acquired brain injury, hospital discharge or short term, crisis intervention work in the future.

The role
Workers on the team have an opportunity to develop their skills and knowledge around a wide range of issues including Care Act Assessments, Mental Capacity Assessments and Best Interest Decisions, Court of Protection work, safeguarding enquiries, and Continuing Health Care processes. You will work within the legislation of the Care Act 2014, Mental Capacity Act 2005, and the Mental Health Act 1983 to deliver a personalised assessment, support planning and reviews following the strengths-based approach, networking with partners and ensuring holistic outcomes.

Education, Qualifications and Knowledge
Recognised social work qualification
SWE registration
Evidence of Continuous Professional Development
Knowledge of the equipment and adaptations criteria, assessment process and provision
Knowledge of relevant legislation, Government policy and guidance in relation to social care, health and housing.
Additional qualifications e.g. Post Graduate Certificate, Practice Educator, BIA, AMHP
37 Hours per week
Flexible, innovative approach and good time keeping
Full and current driving licence
Ability to carry out visits to service users homes and other community venues
Completion of mandatory training
Enhanced DBS clearance
