Summary

Diabetes Specialist Responsibilities Provide evidence-based diabetes assessment, management, and review for patients with Type 1, Type 2, and other forms of diabetes Support insulin initiation, titration, and monitoring in line with local and national guidance Deliver structured patient education, promoting self-management and prevention of complications Work collaboratively with GPs, pharmacists, and community services to optimise diabetes care Identify patients at risk of complications and escalate care appropriately Contribute to audit, quality improvement, and service development within diabetes care General Practice Nursing Responsibilities (Desirable) Undertake cervical cytology (smear taking) in line with national screening programmes Provide wound assessment and management, including leg ulcers and complex wounds Deliver adult and childhood immunisations, in accordance with the national immunisation schedule Provide travel health advice and travel vaccinations, where trained Support the management of other long-term conditions as required Maintain accurate, timely clinical documentation using electronic patient records Leadership, Education and Professional Responsibilities Provide support to nursing colleagues and the wider MDT Participate in the training, mentorship, and development of staff and students Contribute to service planning, evaluation, and improvement initiatives Maintain professional development in line with NMC requirements and Trust policies Demonstrate initiative, flexibility, and problem-solving skills in clinical practice
