Summary
To work as part of Mickleover Medical Centre medicines management team within Greater Derby PCN, supporting the safe, effective and cost-effective use of medicines for the practice population. The post-holder will be employed by the practice and will undertake medicines optimisation, discharge reconciliation, repeat prescribing support, audit activity and patient-facing consultations under the supervision of the Practice Lead Pharmacist and GPs, in line with local protocols, professional standards and governance arrangements. The role is supported by structured supervision and development through the CPPE Primary Care Pharmacy Education Pathway.
