Summary
To triage and manage referrals into the School Nursing Service within an agreed timeframe and forwarded onto teams as required. To support with health care plans. To manage the Paediatric Sleep Pathway with initial assessment and support with sleep diaries. To support to ChatHealth Service. Supporting the Families First for Children Pathfinder Programme (FFCP) with attendance at meetings and contribution to reports and information requests. To co-ordinate movements in and out.
