Summary
To provide training as required. To work collaboratively, and in partnership with, the full range of health and social care providers in Gloucestershire. To provide support and guidance within an inpatient setting if a hospital admission is required To undertake good practice modelling and direct support To undertake care planning To undertake medication monitoring, To provide specialist support and advice in the area of behavioural management and risk management.
