THE ROSEMARY FOUNDATION Role Summary - Clinical Lead Job Title Hospice at HomeClinical Lead Location The Rosemary Foundation (TRF) Reporting to Registered Manager/RN Job Purpose The post holder is a key member of the Senior Leadership Team- Registered Manager, Finance Manager, Operational & Fundraising Manager & Clinical Lead and is responsible for providing clinical expertise, leadership and development to Nurses and HCAs, to ensure delivery of excellence in palliative care services to patients and their families in the community. Key Accountabilities, Responsibilities &Tasks Role Specifics To Lead and Manage the Hospice at Home Clinical Team Take part in the TRF On Call Rota. To support and direct assessment of the care needs of patients and their families referred to TRF. To carry out and / or supervise the carrying out of specific treatments and clinical procedures. To base clinical practice on current evidence-based practice. To oversee the management of effective triage and assessment of new referrals within the Team. Provide leadership to the clinical team engaging with key partners and the wider Specialist Palliative Care MDT Frameworks. Participate and attend Governance and End of Life meetings in support of strong effective relationships with stakeholders. Work effectively with Multi-Disciplinary Teams across care boundaries supporting patients choice to be at home in a safe environment. To be responsible for the clinical team, ensuring effective retention and recruitment of staff and support sickness and well-being management. To maintain continuing personal contact with patients and evaluate the effectiveness of care delivered. To assess the needs of patients and carers requiring intervention from other professionals and liaising as appropriate. To provide advice on symptom control/care management to other health care professionals, domiciliary agencies ,and care homes as appropriate. To support the clinical team to undertake appropriate verification when a death has occurred, notifying relatives and carers as appropriate following TRF policy. Provide high quality timely care for the deceased patient by administering last offices as appropriate and ensuring the safe and timely removal of the body from the home working within the procedures in place within the community. To review, promote and comply with TRF Policies, Procedures & Guidelines PPG To ensure PPGs are regularly reviewed and clearly demonstrate up to date evidenced based practices To understand the obligation to comply with external stakeholder policies. To be responsible for the safety and positive experience of student nurses and others undertaking placements in TRF To assist the Registered Manager in compiling standards to promote a high quality of patient care. To administer drugs and assist with their ordering and storage in the home environment, in accordance with the NMC Standards for the Administration of Medicines. To recognise and report any errors or omissions in the administration of medications. To work to develop quality improvement initiatives with the Senior Leadership Team In conjunction with the Registered manager to support all volunteers and ensure appropriate governance systems are in place Support data collection and provide clinical data reports for the Board of Trustees Oversee the day-to-day management of Systmone, ensuring staff are competent and confident in documenting patient care and interventions Recognise, Respond and Report any safeguarding concerns and escalate accordingly. In conjunction with the Registered Manager report and manage any complaints or safeguarding concerns from patients, their relatives, or others acting on their behalf and assist with investigation of complaints as necessary To support the Fundraising& Operational manager and support the clinical team to contribute Fundraising Events To contribute to the Health & Safety Committee To adhere to the NMC Code of Professional Conduct. To ensure knowledge about patients is maintained in the strictest confidence and is not divulged except to professionals where it would materially assist with the persons care or protection. Qualifications, Skills, Experience, Knowledge & Approach RGN or RN Adult Branch On NMC Register parts 1 or 12 Educated to degree level Evidence of relevant area of study/qualification in Palliative Care Five years' experience in Palliative and/or Community Care 3years proven leadership/management experience Information Technology Skills Evidence of service development and implementation Driving License/Car Owner Desirable Knowledge andexperienceofSystmone Community nursing qualification Teaching experience Skills Excellent basic nursing care skills Excellent communication and interpersonal skills Ability to facilitate individualized evidence-based care Leadership insight and evidence of effective team co-ordination skills Ability to prioritise workload and be flexible to changing demands Ability to share knowledge with others to develop and influence practice Able to maintain active and contemporaneous records Well-developed coping strategies Knowledge Able to demonstrate a good insight into palliative care principles and philosophy Sound pain and symptom control knowledge Communication To communicate effectively with patients, carers and the bereaved to establish and maintain trusting relationships. To ensure communication between members of the hospice at home team is effective so the care needs to patients are easily transferred to the person or persons assuming continuity of care at shift changeovers. Maintain effective liaison with the Community Matrons & Nursing Teams/GPs, Macmillian Palliative Care Team, The Rowans Hospice and Emergency End of Life Rapid Response Team, Hospital Palliative care team, and the Ambulance Service Develop and maintains effective relationships with members of the Primary Health Care Team and all other agencies involved in delivering care in the community. To offer a high level of emotional support to patients and/or carers particularly during the breaking of bad news and around time of death. To contribute to bereavement care offered to families by facilitating bereavement support visits and assessing risk factors for bereavement, referring to TRF bereavement and counselling service, as necessary. To sensitively handle out of hours enquires from patients and carers. To develop and maintain effective working relationships with the Senior Leadership Team, Trustees, Fundraising Team, Administrative staff ,and volunteers. To promote the aims and ideals of The Rosemary Foundation to the public. Internal & External Contacts Internal: Patients, carers, and relatives. All TRF staff, Trustees and volunteers External: Patients, carers and relatives, NHS Trusts, Primary Health Care Teams, other Hospices and Charitable organisations, Nursing & Care Homes, Social Services, Emergency Services, Funeral Directors, Coroner. Decision Making Freedom to make decisions within the boundaries of the job description Responsible for knowing own limitations and when to seek help or advice Mental and Physical Consideration, Working Conditions & Environment Mentally tiring due to prolonged episodes of emotional listening and high levels of support for patients/carers and staff. Regular safe moving/handling of patients and equipment, including beds, mattresses, chairs and as necessary. Regular bending and stretching involved. Health & Safety Contribute towards ensuring that exposure to risk is minimised through safe practice and awareness of health and safety and infection control To ensure that equipment is kept clean and in good working order, checked routinely and that any defect or malfunction is managed. To always act to promote the safety and wellbeing of patients. To ensure the safe storage and use of hazardous substances according to COSHH regulations. To be aware of the security of TRF building out of office hours, alongside the SLT undertake the key holder role. To be aware of security within the patients home To ensure that any observed actual or potential Health & Safety hazards/concerns or issues are immediately reported to the Registered Manager/Operational Manager Work within TRF lone worker policy and ensure staff within the team adhere to the lone worker policy.
