Summary
Main Responsibility Plan, deliver and evaluate clinically appropriate interventions using evidenced based guidance and practice. Set up processes/protocols for effective assessment, planning, implementation and evaluation of care for those patients with physical, mental health and substance misuse. Safeguard vulnerable patients in line with CNWL Trust safeguarding policies. Work with the qualified School Nurse(s) in the team to ensure the children and young people in our care are meeting their expected developmental milestones, alongside health and wellbeing screening assessments. Apply local risk assessment and management. Create and record care plans within individual patient records, tailored to the individual patient. Make appropriate referrals to other services for long term or complex care management. Co-ordinate multi-disciplinary clinical practice to provide focused and integrated care treatment programs/packages. Use highly skilled interventions to de-escalate aggressive patient behaviours and be aware of the appropriate processes for dealing with Work collaboratively with staff of other disciplines e.g. prison, education staff to provide integrated and coordinated Work within own sphere of competency and ensure that training needs are communicated to the team Research and Audits: Utilise theory, evidence-based literature and research to support evidence-based practice. Assist in audits as per the annual planner or when requested to do so in order to develop and improve service provision. Teaching, Training and Supervision: Participate in regular clinical and managerial supervision in accordance with professional practice and CNWL guidelines. Participate in a 3 week supernumerary period, working with RGNs, RCNs and RMNs to begin your understanding of the role being delivered as well as the expectations of you. We offer locally arranged teaching sessions every 2 weeks covering varied clinical skills and knowledge.
