Summary
To provide holistic care for service users with severe and enduring mental health problems as per Dialog plus/ care needs. To support service users on their recovery pathway Work collaboratively with our partners carers. relatives. and other social inclusion Identify risk and manage risk appropriately in the best interest of the service users and others. To discuss cases at the MDT for professional input and support To signpost service users to appropriate services as part of recovery pathways
