This role supports patients mental wellbeing and social health through personalised social prescribing and community engagement, in line with the agreed work plan. Working across Durrington and Upavon, the postholder provides one-to-one consultations, develops action plans, and signposts patients to appropriate services. The role leads on support for carers and military veterans and follows up frequent attenders to improve outcomes and reduce avoidable appointments. Working closely with clinical and non-clinical teams, the postholder maintains accurate records, builds strong community links, undertakes required training and supervision, and contributes to PCN wellbeing initiatives to improve patient experience and access to support.
