Summary
Main duties of the job The post holder will perform tasks prescribed and delegated by the GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and within defined competencies. The main duties of the role include (but are not limited to): Undertake chronic disease management clinics, including Asthma, COPD/Spirometry, Diabetes and Chronic Heart Disease. Identify, and manage as appropriate; treatment plans for patients at risk of developing a long-term condition. Perform basic nursing tasks such as blood pressure monitoring, ECGs, phlebotomy etc. Participate in vaccination and immunisation programmes; administering injections, ensuring safe vaccine storage and implementing cold chain policy. Undertake health screening, surveillance and new-patient screens. Take pathology samples from patients for analysis (blood, urine, cervical samples etc). The role will require flexibility to meet the demands of this large practice and an individual work plan will be agreed with the successful applicant.
