Summary

Duties Specific to Role Leadership PCN diabetes nurse team leadership and supervision. Lead, support and manage other members of the PCN diabetes team, including trainees and HCAs. Direct Patient Care Deliver a dedicated diabetes specialist Lead Nurse clinic targeting patients stratified as Red Risk under the PCNs diabetes risk model. Review complex cases and provide specialist input into the diagnosis, monitoring, and treatment of patients with Type 2 diabetes. Initiate and optimise pharmacological treatment including insulin, GLP-1 receptor agonists, SGLT2 inhibitors, and other oral hypoglycaemic agents, in accordance with NICE and local guidelines. Manage follow-up pathways including blood testing, medication titration, and patient education. Liaise with Diabetes Practices Nurses to determine clinic lists and monitor progress across the cohort. Actively participate in monthly MDTs including integrated neighbourhood working and encourage the PCN to engage in a wider-system approach to Type 2 Diabetes care. Complete the PCN-approved diabetes template on SystmOne for every patient seen. Ensure all consultations, interventions, and follow-up actions are clearly recorded. Code the initiation of medications and all diabetes-related activity accurately using appropriate clinical codes to support safe prescribing, data quality, and audit readiness. Provide diabetes clinical care to patients registered with PCN practices, within your specialist skills and competence. Primary care specialist diabetes nurse duties including (but not limited to) face to face consultations, video consultations, telephone consultations, e-consultations and queries. Clinical administration, and dealing with queries, paperwork and correspondence in a timely fashion. Provision of care for acute and long-term diabetes conditions and contributing to the delivery of QOF and enhanced services. Prescribing in accordance with locally agreed or national guidelines. As a qualified non-medical prescriber (NMP)/independent prescriber (IP), prescribing will be undertaken within the scope of practice of the prescriber and in accordance with their professional code of conduct as set out by their own regulatory bodies and organisational NMP policy Provision of preventative care, including health promotion advice, vaccinations and screening Support of patients in the developing self-management strategies and in fostering patient engagement with the PCN Practices. Working always within your professional competencies, you will make yourself available to undertake a variety of duties including surgery consultations and emergencies; telephone consultations and queries; triaging; and dealing with queries, patient test results, patient medical reports and examinations; and NHS/private paperwork and correspondence. Training and promotion Ensure skills of PCN diabetes nurse team are appropriately and effectively used to deliver care. Support the PCN diabetes team in developing their skills and expertise To be aware of existing skills within the PCN and highlight training needs specific to diabetes services, along with an awareness of training opportunities, locally and nationally. Provide diabetes training and support to nurses within Gloucester Inner City PCN Practices. Work across their PCN to promote awareness and engagement with the best practices for diabetes care, ensuring a consistently high standard across the PCN. Attend all practice PLTs at least once a year to provide diabetes training and update on the PCN diabetes service. Attend PCN Board Meeting annually to provide update on PCN diabetes service. Plan, develop, implement, and evaluate new diabetes health promotion and long-term condition care. Advice and support To be a point of contact for practice staff members regarding delivery of diabetes care. Provide professional advice to practices on diabetes developments, recommending changes to working practices in line with evidence-based knowledge. Lead and monitor the development and implementation of diabetes policies, standards and guidelines across the PCN diabetes team and practice nursing teams. Operational Ensure an effective nurse and appointment system, in collaboration with PCN Business Manager and PCN Project Manager. In collaboration with the PCN Business Manager and the Recruitment Team, manage the recruitment, onboarding, induction and ongoing employment of the PCN diabetes team including nurses, healthcare assistants and health and wellbeing coaches. Monitor and advise on the effective use of physical and financial resources by the PCN diabetes team, in partnership with the PCN Business Manager. Ensure a robust system for PCN diabetes nurse appraisal and revalidation Cover for other diabetes nurses will be provided by reciprocal sharing of correspondence, results and other administration. Quality and Clinical Governance Working with the Clinical Directors and Lead Diabetes GP, develop and monitor key quality markers, thereby promoting clinical effectiveness. Enable the PCN diabetes service to apply and improve clinical governance and risk management systems. Operate in line with national and local diabetes guidelines and PCN protocols. Ensure that you are familiar with CQC, NHS and NICE requirements relevant to the diabetes service and support the team in understanding them. Areas of non-compliance must immediately be escalated to the PCN Clinical Directors. Ensure that you are familiar with the Health and Social Care Act 2008 Code of practice on the prevention and control of infections and related guidance and that the diabetes team is compliant with them. Areas of non-compliance must immediately be escalated to the PCN Clinical Directors. To meet with PCN Clinical Directors, quarterly as a minimum, to ensure appropriate clinical governance and oversight of the service. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Recording clear and contemporaneous I.T. based consultation notes to agreed standards; Collecting data for audit purposes as per Practice protocols. Contributing to patient safety by maintaining good infection control thorough monitoring and maintenance of stock and equipment to include refrigeration, sterilising equipment and emergency equipment. Complying with the Nursing and Midwifery Councils Code and professional standards for nurses. Work in compliance with CQC, statutory and NHS requirements applicable to the PCN diabetes service Complying with the training requirements of G DOC and of the NMC Complying with requirements for revalidation Meetings and collaboration Participation and representation from Primary Care/PCN at events/meetings where requested and appropriate. Participate in pre-clinic meetings and ongoing project reviews, contributing to the evaluation and improvement of the enhanced diabetes service.
