Summary
About the role We are seeking a compassionate, motivated Inclusion Health Nurse to join our team supporting people facing severe disadvantage, including homelessness, insecure housing, and other forms of social exclusion. This is a patient-facing role with real variety. You will support our drop-in service, deliver long-term condition care, and help individuals navigate health needs that are often complex, unmet, and shaped by trauma and inequality. You will work as part of a multi-disciplinary, multi-agency network, providing safe, practical clinical care and building trusted relationships over time. Some outreach work may be required, including working alongside partner organisations in community settings. We are looking for someone who can work autonomously, build trust, and provide consistent, compassionate care. A genuine interest in this patient cohort and a trauma-informed approach are essential. Key responsibilities Support the morning drop-in service, providing assessment, treatment, brief interventions and navigation to services. Deliver inclusion health nursing activity including complex wound care and dressings, injections, and other clinical procedures. Provide long-term condition support and preventative care (e.g. diabetes, respiratory disease, hypertension). Provide harm reduction advice and brief interventions, supporting safer health behaviours and access to wider services. Undertake BBV screening (e.g. blood-borne viruses) and support onward referral and follow-up in line with local pathways and protocols. Substance misuse and OST support (non-prescribing): support patients receiving OST, including administration of long-acting injectable buprenorphine (Buvidal) in line with protocols and competency sign-off. You will not prescribe or clinically manage OST. Training and supervision in substance misuse will be provided. Develop, document and review nursing care plans and clinical management plans, including for complex wounds and longer-term nursing needs, and ensure these are implemented consistently. Provide clinical oversight, guidance and appropriate delegation to the Nursing Associate, ensuring tasks are within scope of practice, supported by clear plans/protocols and supervision, and escalated appropriately when required. Work with partner agencies and contribute to safe, joined-up care. Maintain accurate clinical records and apply safeguarding practice at all times. Support outreach activity where needed to improve access and continuity. Essential criteria Registered Nurse (NMC). Demonstrable experience in complex wound care, including assessment, management planning and follow-up. Able to work autonomously, prioritise effectively, and make safe clinical decisions. Able to develop care plans and provide clinical oversight/delegation where required, including working with a Nursing Associate. Strong communication skills and confidence engaging people with complex needs, using a trauma-informed approach. Commitment to safeguarding and professional standards. Demonstrable interest in inclusion health and reducing health inequalities. Desirable Experience working with inclusion health groups (homelessness, substance misuse, severe mental illness, complex trauma). Long-term condition management experience and/or relevant qualifications. Experience of outreach, assertive engagement, or service navigation support. Experience of multi-agency working and care coordination. Experience using clinical systems (e.g. SystmOne).
