Summary
To undertake clinical supervision to further develop competence and clinical practice. To liaise, report and feedback to the care coordinators / team who maintain clinical responsibility for the cases by participating in regular team meetings and supervision sessions. To record and communicate effectively all clinical information to the care coordinator / MDT and via the Electronic Patient Record system To plan and prioritise your own appointments and workload and to work flexibly depending on the needs of the adult, young person and family. To carry out clinic, home and community visits as delegated by the MDT To attend client related meetings as appropriate. At all times minimises risk to self and others by undertaking safe working practice adhering to the lone working policy.
