Summary
Role Summary The Advanced Nurse Practitioner will provide high-quality, autonomous, patient-focused care across a range of acute, chronic, and complex presentations. The role requires independent clinical decision-making, including assessment, diagnosis, management, prescribing, and referral, while collaborating closely with the wider multidisciplinary team. Key Responsibilities Assess, diagnose, and manage patients presenting with acute, chronic, and minor illness. Independently prescribe medications within scope of practice. Order, interpret, and act on diagnostic tests and investigations. Make appropriate referrals to secondary care and other services. Work collaboratively with GPs, nurses, pharmacists, and other practice staff to ensure continuity and quality of care. Maintain accurate, timely, and comprehensive patient records. Support service development and participate in quality improvement initiatives. Promote patient education, self-management, and health promotion. Clinical Expectations Demonstrate advanced clinical skills and judgment in assessing and managing patients. Maintain up-to-date knowledge of evidence-based practice, clinical guidelines, and professional standards. Practice safely, ensuring patient safety and risk management at all times. Provide mentorship and support to junior clinical staff as required.
